Título:L'influence de la tuberculose pulmonaire sur l'èvolution des lésions cutanées lepreuse.

Resumo:La tuberculose pulmanaire étant fréquente parmi les lepreux, les auteurs ont voulu observer l'influence qu'elle pourrait avoir sur l'évulution des lésions cutanées lépreuse. Les auteurs ont choisi leurs cas de tuberculose, ne s'occupant que de ceux qui avaient au moins un an et demi d'observation. Ainsi, ils ont pu réunir seulement 18 cas, remarquant que chez 13 patients les lésions cutanées se sont aggravées, tandis que chez 4 autres elles restèrent stationnaires, et qu'un seul malade présenta des amélirations de ces lésions. It pensent qu'il ne peuvent pas dégager de conclusions définitives parce que leurs observations ne portent que sur un nombre réduit de cas. Ils cont d'avis que la tuberculose pulmonaire quand elle se manifeste chez un malade lépreux, doid coitribuer á l'aggravation de la lèpre car lórganisme aura besion de mobiliser ses defenses contre plus d'une affection. Ils étudient quel est le mécanisme défensif qui s'établit quand les lésions cutanées lepreuse sont bénéficiées par la tuberculose pulmonaire. Ensuite ils considèrent la possibilité que la tuberculose aurait, d'altérer l'évulution de la lèpre que façon à déterminer l'apparition des lésions tuberculoïdes. Ils prennent comme point de départ l'affirmation suivante de RABELLO JUNIOR: "La sensibilisation par le virus tuberculeux peut influencer la marche dun processus lépreux, ce qui se traduit sous un point de vue statistique... Resumo:Diagnosis of tuberculosis a problem, specially in the regions harboring an abundance of both pathogenic and non-pathogenic mycobacteria. This study was undertaken to assess in such a situation the predictive value of proliferative T cell response to a peptide epitope ('38G') of the 38 kDa membrane protein of Mycobacterium tuberculosis. 3[H]-thymidine incorporation assays were done with peripheral blood mononuclear cells of tuberculoid leprosy and pulmonary tuberculosis patients. The donors were also classified as PPD responders (Stimulation Index, SI> 3) or non-responders (SI < or = 3) on the basis of their T cell response to the 'Purified Protein Derivative (PPD)' of M. tuberculosis. 38G peptide was used in either free or liposome-associated form prepared by the technique of 'Dehydration-rehydration Vesicles' (Kirby and Gregoriadis, 1984). While free peptide failed to induce a positive response in study subjects, its liposomal form was T cell stimulatory and distinguished, to certain extent, between PPD responders (corresponding SI > 3 in 54 per cent subjects) and non-responders (SI > 3 in 29 per cent subjects). However, it did not differentiate between leprosy and tuberculosis. The study supports use of liposomes as adjuvant vehicles for antigenic peptides designed to activate human T cells. (AU).

Resumo:Humans are exposed worldwide to a variety of environmental mycobacteria (EM) and most children are inoculated with live Bacille Calmette-Guérin (BCG) vaccine. Although rarely pathogenic, poorly virulent mycobacteria, including BCG and most EM, may cause a variety of clinical diseases. M. tuberculosis and M. leprae are more virulent, causing tuberculosis, and leprosy, respectively. Remarkably, only a minority of individuals develop clinical disease, even if infected with virulent mycobacteria. There is now accumulating evidence that the large interindividual variability of clinical outcome results in part from variability in the human genes that control host defense. We review here in current knowledge about genetic predisposition to common (leprosy and tuberculosis) and rare (BCG and EM infections) mycobacterial infections. (AU).

Resumo:A relatively mild nonoliguric acute renal failure developed early in the course of daily rifampin treatment in a 54-year-old man who was receiving concomitant steroid therapy. The renal lesion was an acute interstitial nephritis characterized by tubular dysfunction:inability to concentrate urine, increase in fractional excretion of sodium and uric acid, and glycosuria with normal blood glucose values. All abnormalities resolved promptly upon withdrawal of rifampin while steroid therapy was gradually reduced. The documents the fact that rifampin-induced nephropathy can occur in the course of a continuous daily regimen and may be favorably influenced by steroid administration.(AU).

Resumo:It has been assumed in the past that attack-rates for leprosy are lower than those for tuberculosis, that leprosy is spread by direct person-to-person contact, and that in leprosy the skin is the most likely route of infection. However, the numbers of Mycobacterium tuberculosis in sputum from open cases of tuberculosis and of M. leprae in the nasal secretions of patients with lepromatous leprosy are similar; in addition, attack-rates in family or household contacts for the two diseases in comparable populations in South India are of the same orger. While these findings do not prove that modes of spread and routes of infection are identical for the two diseases, they are consistent with this possibility.(AU).
